The Case Manager is your resource. Your connection to finding solutions for the challenges of in-home care. That Case Manager takes the time to get to know you, your loved on and your family and does a comprehensive review of the situation. The idea is to find solutions and the first step is understanding the needs. Call us and we will find a convenient time for you to start.
The In-Home Assessment
A comprehensive review of the entire situation. We come to you and really look at the health and safety of the person in need and help find answers. Often the solution is not something we provide so we help find it for you. Once there is an understanding of the circumstances, a Plan of Care is developed and used as a road map to guide everyone involved. The goal is a better quality of life for everyone involved.
Monitoring and Management
The start of care is just that – a start. We continue to evaluate, monitor, manage, and review all aspects of the case to determine if any changes or adjustments need to be made. In-home care situations tend to evolve over time and require flexibility in how things are handled. Your Case Manager continues to be your resource to ensure the best outcomes.
